چکیده

Headless screws are favored for internal fixation in the treatment of displaced or unstable small bone (e.g., Scaphoid) fractures, because they are embedded below the articular surface of the bone, reducing tissue irritation and immobilization. Compression plays an important role in fracture stability, maintaining gap reduction and also accelerating the healing of cancellous bone [1]. There are several types of screws being used in practice and it is of clinical interest to know how different they are in generating and maintaining the compression force. Several studies have investigated the compressive forces of headless screws [2,3]. However, little attention has been paid to the fastening torque and the effect of pre-drilling. This study compares the generated compression force and fastening torque and the effect of pre-drilling during the insertion of five frequently used headless compression screws that are typically used for scaphoid fixation.

Biomechanical comparison of headless antegrade screw versus retrograde cortical screw for coronoid fracture fixation.

OBJECTIVE Posterior-to-anterior directed screws are stronger than anterior-to-posterior directed screws for coronoid fracture fixation. Anterior approaches that facilitate direct reduction and fixation of coronoid fractures have been described. The present study was based on the hypothesis that anterior-to-posterior headless screw (Acutrak Mini® 3.5 mm × 26 mm, Acumed, Hillsboro, Oregon, USA) f...

Biomechanical comparison of first metatarsophalangeal joint arthrodeses using triple-threaded headless screws versus partially threaded lag screws.

BACKGROUND Triple-threaded, cannulated headless screws of varying thread diameters and pitch are designed to maintain thread length across the arthrodesis plane, provide joint compression, and reduce countersinking. This study tested the biomechanical fixation strength of conventional partially threaded lag screws compared to triple-threaded headless screws in first metatarsophalangeal joint ar...

Internal Fixation of Unstable Osteochondritis Dissecans in the Skeletally Mature Knee with Metal Screws

PURPOSE Several bioabsorbable and metal options are available for internal fixation of an unstable osteochondritis dissecans (OCD) lesion, but currently there are little data on outcomes with metal headless compression screws in the adult knee. The purpose of this study was to determine (1) the radiographic healing rates, (2) midterm clinical outcomes, and (3) comparison between healed and unhe...

Bicondylar Hoffa Fracture Successfully Treated with Headless Compression Screws

Bicondylar coronal plane fracture, eponymically named Hoffa fractures, is an extremely rare injury. We present a case of isolated unilateral bicondylar Hoffa fracture that was successfully treated with open reduction and internal fixation using headless compression screws with satisfactory results. We inserted posteroanteriorly oriented Acutrak screws perpendicular to the fracture plane via lat...

First Metatarsophalangeal Joint Arthrodesis: A Retrospective Comparison of Crossed-screws, Locking and Non-Locking Plate Fixation with Lag Screw

Background:Locking plate fixation is increasingly used for first metatarsophalangeal joint (MTP-I) arthrodesis. Still there is few comparable clinical data regarding this procedure. Methods:We retrospectively evaluated 60 patients who received an arthrodesis of the MTP-I between January 2008 and June 2010. With 20 patients each we performed a locking plate fixation with lag screw, arthrodesis w...
